§ 22-75. Claim for compensation.

(a)

Filing. All claims shall be filed with the City Manager in person or by United States mail. The filing date is the date the claim is received by the city.

(b)

Submittal requirements. All claims:

(1)

Shall be submitted for review upon forms established by the city and must include all information as required by ORS 195.312; and

(2)

Must be accompanied by a fee in an amount as set by Council resolution.
